The Occupational Safety and Health Administration (OSHA) began its inspection and investigation the following day and issued a Citation and Notification of Penalty (Citation) to LyondellBasell on January 1, 2022, alleging serious violations of the Occupational Safety and Health Act of 1970, 29 U.S.C. §§ 651-678 (Act). Specifically, the Secretary alleges LyondellBasell exposed workers at the LaPorte facility “to process materials, toxic release, and flammable hazards” in violation of the Process Safety Management (PSM) and Lockout/Tagout (LOTO) standards.
