On January 29, 2016, Compliance Safety and Health Officer John Ammon conducted an inspection of the Hardesty oil well site, located in Grady County, Oklahoma. (Tr. 25). The inspection included multiple employers working at the location, and was conducted under a regional emphasis program for upstream oil and gas activities, particularly those involving well sites.
